York Hospital Home Care is a non-profit, Medicare Certified home health agency providing a range of skilled home health care to promote independence, well-being, and quality of life at home.
We provide nursing, physical therapy, occupational therapy, speech therapy, social workers, and home health aides. Our team has the clinical knowledge and expertise of a progressive home health agency with the caring and compassion that York Hospital has long been known for.

Our team helps to close the gap in the continuum of care by providing compassionate and quality services to enable patients to stay in the comfort of home during times of recuperation, disability, or chronic illness. Patient care is individualized and communicated throughout the team. We work closely with a network of community programs with the goal of keeping the individual in their home for as long as possible.

Rehabilitation services
We provide physical, occupational and speech therapy services for patients recovering from joint surgery, traumatic injury, cardiac surgery, and neurological or other conditions at both an acute and chronic level. Our experienced clinicians work with patients to develop a plan of care to help improve safety, strength, and balance, and to maximize mobility.
Home health aide services
Our team can provide help with basic activities of daily living such as bathing, dressing and exercising.
Home care referrals
York Hospital Home Care is not limited to York Hospital patients. The Home Care team accepts referrals for evaluations and skilled home care, regardless of health care system or hospital, for all patients living in York, York Beach, York Harbor, Cape Neddick, Eliot, Berwick, North Berwick, South Berwick, Kennebunk, West Kennebunk, Kittery, Kittery Point, Ogunquit, Sanford, and Wells (including Moody).
York Hospital Home Care provides services 7 days a week. An intake nurse is available to help with patient care referrals Monday through Friday, 8 a.m.–5 p.m. and on-call nurses are also available 24 hours a day.
